During your first visit you will be asked to fill out a comprehensive patient form to better understand your dental needs. To save you time, you are welcome to download and bring the completed form with you to your appointment.
Infection control has always been a top priority for our practice. We follow infection control recommendations made by the American Dental Association (ADA), the Center for Disease Control and Prevention (CDC), and the Occupational Safety and Health Administration (OSHA) to protect our patients and staff. Everything is disinfected from door handles and light switches, to counter tops and contact areas.
Due to the COVID-19 pandemic we are taking extra steps to protect you and our staff. We would like to share with you some of the changes our office has made.
Our office will communicate with you before your appointment to ask COVID-19 related questions.
We will take your temperature and ask you to use hand sanitizer upon entering the office.
Appointments will be managed to allow social distancing between patients.
If you arrive early to your appointment, you might be asked to stay in your car until you receive a phone call or a text from us.
Magazines and books will be removed from the waiting room as they are more difficult to disinfect.
Please wear a face mask upon entering the office.
